The first day of school is just around the corner! To help your family be ready on day one, we’ll be hosting FREE immunization clinics and sports physicals in partnership with Legacy Health and Family Medicine of SW Washington.

Free Immunization & Sports Physical Clinic

📍Roosevelt Elementary School Gym (2921 Falk Rd, Vancouver)

Schedule an appointment (walk-ins are welcome)

Please bring existing immunization records.

If you’re unsure of your child’s immunization status, their records can be checked online if they have been immunized in Washington State. Sign up to access their records here.

Please note: When you go to make an appointment, it shows Legacy’s address. This is not where the clinic will be held. The clinic will be held at Roosevelt Elementary School.

Free Sports Physicals For Uninsured Students (Grades 6-12)
